Multimedia Lab Reservation Request for DU Course

This form must be filled out by the class instructor, not department heads. ONLY use this form to request the lab for official DU courses. Requests are filled on a first come, first served basis. Our phone is 303-871-3595. Requests must be submitted 24 hours PRIOR to start date of the request.
NOTICE: If you need to use software that is not already available in the lab, refer to our Software Installation page. All software installations must be directed through Multimedia Services at least 4 weeks prior to your lab reservation.

By submitting this request you agree to be responsible for the lab and understand the rules and procedures. ABSOLUTELY NO FOOD OR DRINK INSIDE LABS!! There are NO EXCEPTIONS! Remind your students that food and drink is NOT permitted in the labs. If any evidence of food or drink or an unreasonable mess is left, your department will be notified and will be charged a $50.00 fine.
